Make the Diagnosis: Eeesh….When Something Goes “Pop” While You’re Weightlifting

Presentation

A 48-year-old man presented to the doctor with complaints of elbow pain that developed on his right arm after weightlifting earlier that day. He was performing a biceps-strengthening exercise with a weighted barbell when he felt and heard a popping sound and experienced pain around his right elbow. On examination, diffuse ecchymoses at the medial arm and a reverse “Popeye” deformity of the arm were observed. X-ray examination ruled out fractures and avulsions. A positive hook test demonstrated a complete tear, and there was weakness with elbow flexion and supination. The patient denied anabolic steroid use. The patient was healthy and had no relevant medical history. Can you diagnose this patient?
